New Employees coming on Western Mutual Insurance
For employees that are eligible after employer requirements to come on your
group health insurance plan, they need to fill out both:
enrollment card
enrollment form health statement
Employee Changes
If an employee wants to remove or add dependents to their current coverage they
will need to fill out a:
change in status

In order to add dependents to an employee coverage after they're enrolled, there
has to be a qualifying event.  For example: marriage, birth, adoption, loss of
coverage somewhere else.  Any changes have to be submitted to Glacier
Insurance Strategies or Western Mutual Insurance within 30 days of the event.

Claims Forms
Insurance claims for Western Mutual Insurance require forms to be filled out in
certain situations.
The first claim of every year, you will be required to fill out a coordination of benefits
form


If your claim is due to an accident or Western Mutual requests the information, you
will need to fill out an accident form
