| If an employee wants to remove or add dependents to their current coverage they will need to fill out a: change in status In order to add dependents to an employee coverage after they're enrolled, there has to be a qualifying event. For example: marriage, birth, adoption, loss of coverage somewhere else. Any changes have to be submitted to Glacier Insurance Strategies or Western Mutual Insurance within 30 days of the event. |
| Claims Forms Insurance claims for Western Mutual Insurance require forms to be filled out in certain situations. The first claim of every year, you will be required to fill out a coordination of benefits form If your claim is due to an accident or Western Mutual requests the information, you will need to fill out an accident form |
